Delivering digital care services to support chronic disease patients and doctors with disease prevention and these diseases’ complications prevention.
Project start date : 10/11/2020
Last updated : 23/11/2023
Beneficiary country : Viet Nam
What problem does the initiative address ?
There is a high diabetes complications rate in Vietnam at 55%, costing payers twice on an annual basis. This could be due to no screening, late treatment, lack of an integrated care model, or patients’ lack of knowledge about diabetes self-management.
Detailed description of the initiative
In Vietnam, doctors have to take full responsibility to educate their patients on diabetes self-management. With 3-5 minutes for each doctor’s visit, it’s impossible for any doctor to provide adequate assistance to their patients.
DIA-B’s current key product is a 10-week lifestyle change program designed for 18 million diabetes and prediabetes patients in Vietnam.
DIA-B partners with traditional healthcare organizations to provide diabetes self-management education programs or lifestyle change programs, which emphasize healthy self-care practices and treatment adherence, to patients.
These programs have been clinically proven to better manage diabetes and reduce the risk of a patient developing complications. This will help delay the onset of complications or prevent complications among diabetes patients.
What is the proposed solution added value ?
Doctors from these healthcare organizations and DIA-B’s lifestyle specialists are in charge of multiple groups of patients, facilitating lasting healthy lifestyle change at home for these patients.
With the knowledge and skills obtained from these programs, these patients are better equipped to cooperate with their doctors during treatment.
